Medical Statement
This is the Medical Statement required for any training with Bluewater Scuba. If any of the questions are answered "YES", we will need a doctor to sign off on page 2 before we can begin the training. If everything is answered "NO", a doctor's release is not required. Please print this and fill it out. Answer each question "YES" or "NO", not "N" or "Y".
